  2. To identify the challenges and barriers hindering the aggressive implementation of the NPHCD in Nigeria.
  3. To examine the potential impact of a successful, aggressive implementation of the NPHCD on healthcare delivery in Nigeria.

LITERATURE REVIEW

Conceptual Review

National Primary Health Care Development Policy (NPHCD)

The National Primary Health Care Development Policy (NPHCD) is a cornerstone of Nigeria’s healthcare system. Introduced in 1987, it serves as a comprehensive framework designed to address the myriad challenges facing healthcare delivery in the country. The policy, outlined in the National Strategic Health Development Plan II (Federal Government of Nigeria, 2021), aims to provide primary healthcare services to all Nigerians, with a strong emphasis on community participation, equity, and accessibility.

The NPHCD sets forth several critical objectives, including improving the healthcare infrastructure, enhancing healthcare personnel capacity, and ensuring the accessibility of healthcare services. It prioritizes a bottom-up approach to healthcare delivery, focusing on strengthening primary healthcare facilities and making them the first point of contact for healthcare services (National Primary Health Care Development Agency, 2022). The policy recognizes that robust primary healthcare is essential for achieving improved health outcomes and reducing disparities in healthcare access, as highlighted in Cu et al.’s scoping review (2021).

The NPHCD plays a pivotal role in shaping the Nigerian healthcare landscape. It envisions a healthcare system where every citizen has access to quality healthcare services, regardless of their location or socio-economic status (World Health Organization, 2012). The policy aligns with global healthcare goals, such as Universal Health Coverage, as advocated by the World Health Organization (2012), and underscores the importance of a well-functioning primary healthcare system in achieving these objectives. The aggressive implementation of the NPHCD, as underscored by its minimum standards (National Primary Health Care Development Agency, 2022), is essential to address the challenges facing healthcare delivery in Nigeria, as discussed in Amata’s budget analysis (2022).

However, despite its potential to transform healthcare delivery, the NPHCD’s aggressive implementation has been lacking, resulting in persisting challenges in the healthcare system. This study seeks to shed light on the reasons for this implementation gap and the impact it has on healthcare delivery in Nigeria. It is imperative to understand the critical role of the NPHCD, its objectives, and its potential for improving the healthcare system to address the healthcare challenges in the country, as discussed in the guidelines for the administration of the Basic Health Care Provision Fund (2016).

Healthcare Delivery

Healthcare delivery is a multifaceted concept integral to the well-being of any society, including Nigeria. It encompasses a wide array of components, signifies a fundamental pillar of societal development, and is riddled with various challenges.

At its core, healthcare delivery includes the provision of healthcare services, prevention, promotion, treatment, and rehabilitation, as defined by the World Health Organization (2018). These services range from primary care at the community level, as stipulated in the National Primary Health Care Development Policy (Federal Government of Nigeria, 2021), to specialized tertiary care at advanced healthcare facilities. The seamless coordination and integration of these services are essential for a holistic approach to healthcare delivery, as discussed in the Donabedian Model (Murray & Frenk, 2020).

The significance of healthcare delivery cannot be overstated. It directly influences the well-being and productivity of a population, as highlighted in the World Bank’s report (World Bank, 2022). Access to quality healthcare is a fundamental determinant of health outcomes, which, in turn, impacts the quality of life for individuals and the overall productivity of a nation. A well-functioning healthcare system, addressing barriers to healthcare access (Cu et al., 2021), contributes to the social and economic development of a country.

However, healthcare delivery in Nigeria is beset by various challenges, including inadequate healthcare infrastructure, as noted in the World Bank’s report (World Bank, 2022), and a shortage of healthcare personnel, as highlighted by Adedini et al. (2014). These challenges hinder the quality and accessibility of healthcare services, especially in rural areas (Cu et al., 2021). Additionally, fragmentation within the healthcare system (World Health Organization, 2012) and underfunding (Amata, 2022) further exacerbate the complexities and challenges in delivering healthcare.

Understanding the comprehensive concept of healthcare delivery and the challenges it faces is critical in addressing the issues plaguing the Nigerian healthcare system. This study seeks to examine these challenges and their impact, with a particular focus on the potential role of the National Primary Health Care Development Policy (NPHCD) in mitigating them. By exploring the components, significance, and challenges of healthcare delivery, the research aims to provide a holistic understanding of the context in which the NPHCD operates and its potential for transformative change in Nigeria’s healthcare landscape.

 

CHAPTER THREE

RESEARCH METHODOLOGY

Introduction

The methodology section of this research outlines the systematic approach employed to fulfil the study’s objectives. This chapter provides a comprehensive insight into the research design, population, sampling technique, data collection methods, data analysis, validity and reliability measures, and ethical considerations. The chosen methodology aims to address the research questions and objectives effectively, building on a foundation of recognized research philosophies and approaches (Saunders, Lewis, & Thornhill, 2016, 2019).

Research Design

The research design chosen for this study is quantitative, and this selection is underpinned by the necessity to acquire structured data from a substantial sample size, enabling the rigorous statistical analysis of the multifaceted factors impacting healthcare delivery in Nigeria. Quantitative research offers a systematic and methodical approach that aligns with the study’s objectives. It facilitates the comprehensive collection of numerical data, allowing for the identification of patterns, trends, and relationships within the dataset, which is crucial for a robust analysis (Saunders, Lewis, & Thornhill, 2019).

One of the key strengths of the quantitative survey research design lies in its ability to capture a wide array of responses from a sizable sample. In this study, the broad scope of participants, including healthcare professionals, policymakers, and the general population, necessitates a method that can efficiently collect structured data from a diverse range of individuals. Quantitative surveys, with their standardized questionnaires, enable the systematic gathering of data from a considerable number of respondents, ensuring a comprehensive representation of the various perspectives related to healthcare delivery in Nigeria.

Furthermore, the use of structured questionnaires as the primary data collection method within the quantitative research design is highly advantageous. These questionnaires are meticulously designed to cover a spectrum of factors affecting healthcare delivery. They provide a standardized framework for gathering data, ensuring consistency and comparability across responses. The structured nature of the questionnaires streamlines the process of data collection and makes it conducive for quantitative analysis, as the data collected is in a numerical format, ready for statistical scrutiny. This approach is in line with the study’s objective of deriving statistically significant conclusions from the data.

In essence, the selection of a quantitative survey research design, supported by structured questionnaires, is instrumental in achieving the study’s objectives. It offers a systematic and efficient means of collecting data from a diverse and substantial sample, facilitating rigorous statistical analysis. This design choice is grounded in recognized research methodologies and aligns with the research philosophies and approaches to theory development outlined in the literature (Saunders, Lewis, & Thornhill, 2019).

Summary of Findings

The study’s findings provide significant insights into the state of healthcare delivery in Nigeria, focusing on various factors impacting the National Primary Health Care Development (NPHCD) policy’s implementation and effectiveness. This comprehensive investigation involved 120 respondents, including healthcare professionals, policymakers, and the general population, with diverse backgrounds and perspectives. The research was conducted using structured questionnaires, and data analysis was performed using SPSS 27. The study also ensured data validity and reliability by employing Cronbach’s alpha coefficient and maintained ethical considerations throughout the research process.

The first aspect explored in the study was the level of implementation of the NPHCD policy in Nigeria. The respondents’ opinions indicated an overwhelmingly positive outlook, with 95% of them agreeing or strongly agreeing with the statement regarding the NPHCD’s successful implementation. This reflects the perceived impact of the policy in improving healthcare delivery in Nigeria, aligning with its objectives to enhance primary healthcare services. These findings emphasize the need for continued commitment to the NPHCD’s implementation to further strengthen the healthcare system in Nigeria.

The study also delved into the challenges and barriers hindering the aggressive implementation of the NPHCD in Nigeria. While the responses revealed that nearly 90% of the participants acknowledged the existence of challenges, this also demonstrated a strong commitment to addressing these issues. Respondents expressed their concerns, indicating that although the NPHCD has made strides, several obstacles remain. These findings highlight the importance of identifying and tackling these challenges to maximize the policy’s potential and create a more efficient healthcare delivery system.

Conclusion

In conclusion, the study’s findings, based on the hypotheses tested, shed light on the state of healthcare delivery in Nigeria and the implementation of the National Primary Health Care Development (NPHCD) policy. The study collected data from 120 diverse respondents, including healthcare professionals, policymakers, and the general population, who provided valuable insights into the current landscape of healthcare in Nigeria.

The results from the first hypothesis, which assessed the extent of the NPHCD’s implementation in Nigeria, overwhelmingly indicated that the policy has been well-implemented. With 95% of respondents agreeing or strongly agreeing with the statement, this highlights the significant strides made in enhancing primary healthcare services through the NPHCD. This finding underscores the policy’s positive impact on healthcare delivery in Nigeria and emphasizes the need for its continued implementation.

The second hypothesis explored the challenges and barriers hindering the aggressive implementation of the NPHCD. The results revealed that while there are challenges, a high percentage of respondents acknowledged their existence (almost 90%). This finding reflects the commitment to identifying and addressing these obstacles to further strengthen the healthcare system. It highlights the need for proactive strategies to overcome these challenges and maximize the policy’s potential.

The third hypothesis, which focused on the potential impact of a successful, aggressive implementation of the NPHCD on healthcare delivery in Nigeria, indicated that the majority of respondents (82%) recognized the positive impact of the policy. However, some expressed uncertainties and disagreements, emphasizing the complex nature of healthcare delivery in Nigeria. These findings underline the importance of strategic planning and continued research to ensure the policy’s positive effects are optimized.

In summary, the study’s results reinforce the significance of the NPHCD in improving healthcare delivery in Nigeria. They also stress the importance of addressing the challenges that hinder its aggressive implementation and emphasize the need for collaborative efforts among stakeholders to optimize the positive impact of the policy. These findings provide valuable insights for policymakers and healthcare professionals and contribute to the ongoing enhancement of healthcare services in Nigeria.

Recommendations

Based on the findings of the study and the complexities of healthcare delivery in Nigeria, several recommendations can be made to enhance the healthcare system and the implementation of the National Primary Health Care Development (NPHCD) policy:

  1. Strengthen Implementation Oversight: To maintain and further improve the NPHCD, there should be rigorous monitoring and evaluation mechanisms in place. Continuous assessment of policy implementation will help identify areas that need improvement and ensure that the policy’s objectives are consistently met.
  2. Address Existing Challenges: Given that a significant percentage of respondents recognized challenges to the aggressive implementation of the NPHCD, it is crucial to address these barriers proactively. Policymakers should collaborate with healthcare professionals, researchers, and other stakeholders to develop strategies aimed at overcoming these challenges, thereby promoting effective healthcare delivery.
  3. Enhance Public Awareness: To ensure the successful implementation of the NPHCD, it is essential to increase public awareness and engagement. Public awareness campaigns can educate the general population about the benefits of the policy, their rights to quality healthcare, and how they can contribute to its success through active participation.
  4. Collaborative Efforts: A multi-stakeholder approach is vital for the success of the NPHCD. Policymakers, healthcare providers, and the general population should collaborate closely to create a coordinated healthcare system. Efficient communication and collaboration among healthcare providers, as well as between healthcare providers and policymakers, can significantly improve healthcare service delivery.
  5. Continued Research and Adaptation: The complexities of healthcare delivery in Nigeria suggest that research should remain a fundamental component of the healthcare improvement strategy. Future research should explore the evolving needs of the healthcare system, adapt policies to changing circumstances, and contribute to the ongoing enhancement of healthcare services.

Contribution to Knowledge

This study makes a significant contribution to the existing body of knowledge in several ways. Firstly, it offers an in-depth examination of healthcare delivery in Nigeria, shedding light on the challenges, perceptions, and experiences of various stakeholders, including healthcare professionals, policymakers, and the general population. The findings provide a comprehensive understanding of the current state of healthcare in Nigeria, which is essential for both academic research and practical policy improvements. As Nigeria strives to enhance its healthcare system and bridge existing gaps, this research acts as a valuable baseline, serving as a reference point for future studies and interventions aimed at addressing the identified challenges.

Secondly, this study enriches the discourse on the National Primary Health Care Development (NPHCD) policy, a critical framework designed to transform healthcare delivery in Nigeria. By examining the extent to which the NPHCD has been implemented and identifying the barriers hindering its aggressive implementation, this research offers insights into the policy’s effectiveness and the challenges policymakers need to tackle. This knowledge can guide future policy development and reforms to ensure the NPHCD realizes its intended objectives. In summary, this study’s findings enhance our understanding of healthcare delivery in Nigeria and contribute to the development of evidence-based strategies to improve healthcare services and outcomes in the country.
